   Hi,

   We have uploaded the necessary order form, here's the link.
   What you need to do is send us on via email or fax your enrollment details ( college id, enrolement slip etc.), we'll confirm via email that your eliglbe and then we can process your order over the phone or by fax. The details on the order form.
